Fax completed MI Apps to your local Underwriting Service Center OR complete the form on screen, save it for your records and send it as a loan file attachment to The Loan Center (login or learn more).

HARP Loans

HARP eligible loans should be submitted under MGIC's Refi-to-Mod Program using these forms:

Same Insured/Servicer Refi-to-Mod Request

New Insured/Servicer Refi-to-Mod Request

Fax completed Refi-to-Mod forms to your local Underwriting Service Center.

Notice of Loan Modification

If you are unable to submit loan modifications online, use the pdf version of the Notice of Loan Modification form (06/09). Complete the form on screen or by hand, then fax or mail it to MGIC. (Fax number and address are provided on the form.)
