File a claim for mortgage insurance benefits any of these ways — choose the one that best works for you:

• online, via MGIC/Link Servicing: Login or learn more (.pdf)
• electronically, via industry-standard formats such as X12. Contact ecommerce_services@mgic.com.
• by faxing a Claim for Loss (.pdf) form to 1-800-353-8781
• by mailing a Claim for Loss (.pdf) form to MGIC, P.O. Box 525, Milwaukee, WI 53201
